Documented medical conditions that prevent safe administration of immunizations can be documented on the Annual Health and Medical Record. We work hard to keep our Scouts and other program participants safe during our events, but sometimes injuries happen.  Scouts are required to complete an Annual Health & Medical Form every year so that our Leaders have the information and authorization they need to address any health issues that may arise. Where can I find the date of my Scout’s latest BSA Health Form?

The effective date for your Scout’s latest BSA Health Form is tracked in Scoutbook: Navigate to Scoutbook. Click on your My Dashboard. Click on your Scout’s Name under Administration. (You may need to click on Administration to show your Scout.) Scroll down your Scout’s profile and click on Edit Profile (It will appear below the “Awards” section). You will find the date of your Scout’s health form in the field titled Annual Health Record A/B Date. Updated for 2022. Once a year, we ask our families to renew their Scout’s membership to Pack 457.  This helps us to ensure we are: Collecting annual dues from our members. Gathering our Member’s required Health and Medical forms. Only paying national dues for youth who continuing with the program. Only communicating with families who wish to continue in the program.
